PRP WORKSHEET INSTRUCTIONS

 

Part 2: Note Completed Items Instructions:  1. Place a mark corresponding to each completed item in the applicable column above (Part 2). For example, if you said five prayers, note five (or ) in the "prayers" column of Part 2. Note: Include only those items you performed especially for the PRP. Do NOT include items you would have performed anyway. 2. At the end of the year indicated above (on 11/1), total your efforts for each column (prayers, Masses, etc.) & place the totals in the boxes at the bottom of Part 2.

 

Part 3: Report Your Efforts to Us (optional) Instructions:  If you would like your efforts included with the Purgatory Release Project Tally, complete the submission form above and submit it to us according to the instructions below:  Note: Please follow all instructions carefully so that your efforts may be included in our tally.  1. Detach submission form on the dashed lines. DO NOT SUBMIT ENTIRE PAGE! 2. Enter name, year-end date, and location (optional) on the appropriate lines of the submission form.  3. Transfer your total quantity of completed items (from Part 2) to the applicable boxes on the submission form for prayers, Masses, Plenary Indulgences, and other good works. * Please write legibly - Do NOT spell out numbers. If none, indicate "0". * Put quantity ONLY - Do NOT include any other information or detail. 4. Mail your submission form no later than 11/7 of the year indicated to:      Attn: PRP / MyCatholicSource.com, B.F.S., P.O. Box 4292, Rancho Cucamonga, CA 91729-4292 * Do NOT use any other means of reporting than our official submission form. * Do NOT fold excessively, tape, staple, paperclip, etc. your submission form. * Do NOT submit via e-mail. * Do NOT include any other information, questions, comments, or other items with your submission form. * If submitting more than one form in a single envelope, do NOT staple or otherwise attach them. * We may keep or discard any items we receive. * Forms received after 11/15 may NOT be included in tally. * If all instructions are not followed accurately, we may dispose of your submission form without including it in our tally. * Illegible / questionable / offensive / or any other items we deem unsuitable for any reason whatsoever may NOT be included in our tally.  * There is no guarantee that any submission will be included in our tally. * We may include your name and/or location in our tally. If you    do not want your name included, use initials or put "anonymous". You may leave the location field blank if desired. * We may report any details concerning your submission on our site(s), in our newsletter(s), and/or elsewhere, without limit, by any method(s) of our choosing (written, oral, etc.). * Be sure to include full postage. We do NOT pay any postage. * Send via postal mail only. Do NOT require a signature from us. * We are not responsible for lost, misdirected, late, etc. mail. * Do NOT correspond to us concerning your submission.  * Do NOT request any response from us. * Do NOT submit more than one form per person per year. * We make no guarantees whatsoever concerning the Purgatory Release Project (PRP). Participate at your own risk. * The PRP is subject to change, discontinuation, etc. at any time without prior notice. * It is not necessary to report your efforts to us. Although unreported items cannot be included in our tally, your efforts still help the poor souls.
